Tales from The Wood: blank character sheet

The printed copy of Tales from The Wood did not come with a character sheet - it refers you to a "separate file." I emailed the designer and he apologized (the language is from the pdf version, of course) and sent me this, giving me permission to post it here. He hopes to correct the omission in future copies of the printed book, at least providing a character sheet file for free with the book order.
